SDC Participant Release and Waiver
Thank you for participating! Please think safety first, but in case you are injured it is important to know that you will be responsible for any required medical care, and you are expected to have your own medical health insurance policy in place. We need you to sign the waiver below that states you understand this.
To the fullest extent allowed by applicable law, participant, on his/her/their own behalf and on behalf of his/her/their beneficiaries, heirs, executors, and anyone else claiming by or through participant, does hereby release and forever discharge and hold harmless The Street Dog Coalition its officers, directors, employees, volunteers, agents, (“Street Dog Coalition” or “SDC”) and participating community partners.
Participant understands that this release discharges The Street Dog Coalition and participating community partners from any liability or claim that the participant may have against The Street Dog Coalition and/or participating community partners with respect to any bodily injury, personal injury, illness, death, or property damage that may result from participant's participation in the activities, whether caused by the negligence of The Street Dog Coalition or a participating community partner. If any portion of this Agreement is invalid, the remainder will continue in full legal force and effect. Participant also understands that The Street Dog Coalition and participating community partners do not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, workmans comp or disability insurance in the event of injury or illness or any other loss.
Participant agrees that they currently have his or her own Medical Health Insurance Policy in effect at the time of the event. This waiver shall be in effect for all Street Dog Coalition events that the participant participates in.
Foster parents for nonhuman animals (animals) and organizations that operate foster programs for animals or that allow foster programs of nonhuman animals, release SDC employees, successors, assigns, directors, officers, or agents from any, and all liability arising from the foster of a temporary foster care animal. If such animal should harm anyone, or cause damage to the foster parent’s property or organization that operates or has a foster program, he/she/it agrees to use his/her/it homeowner’s insurance, property insurance or other means for any reimbursement for such harm.
It is recommended that the participant has been vaccinated for rabies and titer checked within the last two years or consulted with a personal physician/medical professional to understand and accept any risk of not being vaccinated.
By completing the form below, you are electronically signing this waiver and agree to the terms and conditions outlined above.
